Concierge Care/Direct Primary Care

Direct Primary Care (DPC) is a trending, innovative payment model improving access to affordable healthcare with a simple, flat, transparent membership fee. No fee-for-service payments. No third party billing. It is a model that fosters the enduring and trusting relationship between a patient and his or her primary care provider. Patients have extraordinary access to the provider of their choice and physicians are accountable first and foremost their patients.

ANNUAL PLAN

Fee Includes:

Annual Comprehensive History& Physical Exam and Individualized Preventive Services Recommendations
Ultrasound Screening Exam of Carotids, Thyroid, Heart, Aorta, Kidneys, Liver, Pancreas, Spleen, Bladder, Prostate or Uterus/Ovaries
Annual Lab Tests Drawn Onsite
Follow Up Office Visits by Appointment
Virtual Appointments
Access to provider via cell phone/text/email

Office Visits are BY APPOINTMENT ONLY.

Practice size is limited.

The providers will not bill your insurance plan separately.

Fees effective 12/1/2024

This care model is not an insurance policy. You may still want to have an insurance policy for emergencies, hospital visits and catastrophic illness. This plan does not cover the cost of any additional labs or labs ordered by other providers. This plan does not cover the Fees of Specialists to whom you have been referred or Tests/Procedures at other Facilities. We do not provide care for children under age 19. We are no longer able to offer payment plans.
